About the Role
As a Support Broker, you’ll help adults with developmental disabilities shape and manage services that support their independence and quality of life. You’ll collaborate directly with individuals, families, and community agencies to ensure services match each person’s goals under Michigan’s Self‑Determination model.
This role is ideal for someone early in their human‑services career (e.g., recent social work or psychology graduate) or an experienced professional who values autonomy, smaller caseloads, and meaningful one‑on‑one work.
What You’ll Do

Partner with individuals and families to develop and monitor person‑centered support plans.

Coordinate communication among providers, families, and community partners to ensure quality care.

Maintain accurate documentation that meets Medicaid and agency standards.

Support families in navigating systems and advocating for participant choice.

Participate in paid training sessions to strengthen skills and maintain compliance.
Schedule and Work Environment
Average 30–35 hours per week, primarily Monday–Friday, with flexibility to schedule visits based on participant needs.
Mix of community visits, remote documentation, and meetings. Typical travel is within [County/Region].

What You’ll Do
