What Does a Support Broker Do?

The Support Broker service offers an additional option for individuals seeking to implement the principles of Self-Determination. This service complements, rather than replaces, other available service options. A Support Broker serves as a consultant dedicated to enhancing your understanding and collaborating with you to create processes that empower you to effectively exercise Self-Determination while adhering to federal and state regulations. The roles of a Support Broker are tailored to your personal goals and objectives outlined in your Individual Plan of Service (IPOS). 

Examples include:

  • Assisting with planning and/or facilitating planning using person-centered principles

  • Developing an individual plan of service using the person-centered planning process

  • Linking to, coordinating with, follow-up of, advocacy with, and/or monitoring of Specialty Services and Supports and other community services/supports

  • Brokering of providers of service/supports

  • Assistance with access to entitlements and/or legal representation

  • Coordination with the Medicaid Health Plan, Medicaid fee-for-service, or other health care providers

  • Assistance with exploring the availability of community service and supports, housing, and employment and making necessary arrangements to link the Individual with those supports

  • Providing education, training, and technical assistance to the individual and his/her Circle of Support/Planning Team in implementing the Self-Directed Budget according to applicable federal and state standards

  • Monitoring Self-Direction expenditures to ensure that spending does not exceed the Self-Directed Budget by assisting the individual and the Circle of Support/Planning Team to review the expenditure report provided by the Fiscal Intermediary (FMS)

  • Assisting the individual with properly documenting services according to applicable federal and state regulations and policy (including assisting the individual with reviewing and submitting documentation to the FI, such as: employee time sheets; monthly summary note; mileage and expense reimbursement forms; and all other required documentation) and

  • Assisting the individual with hiring and retaining appropriate support staff (including assisting the individual in recruiting, interviewing, hiring, scheduling, and supervising self-hired staff, and assisting the individual with identifying and retaining adequate backup staffing to ensure availability as needed by the individual)
